My Future, My Plan

A transition planning resource that includes a DVD video and discussion guide, as well as a Student Manual and a Family/Teacher Guide. These are designed to inform students with disabilities and their families about the transition planning process and encourage them to use this process to achieve their goals for life after high school. This resource can be used to engage groups of students with disabilities, their families, and professionals in the transition planning process.
